A place to begin
Hear the early signal, then separate facts, uncertainties, likely impact and the time for a decision. Notice whether raising a risk leads to shared judgment or mainly to blame and extra reporting for the person who spoke.
Why share before certainty?
A colleague reports a possible delivery risk. You may ask for stronger evidence first or why it was not prevented. The wish to judge accurately is understandable. Also consider when that colleague might speak next time after experiencing your response. Explore what needs deciding together while the information is still incomplete.
Distinguish facts and possibilities
What has been confirmed and what remains a forecast? Who could be affected, and when would a decision preserve options? You need neither treat uncertainty as certainty nor close the conversation until every fact is known. Questions can help find the context for a choice rather than serve as a test of the person raising the concern.
Do not return every burden to the messenger
If speaking up repeatedly means owning the entire solution, people may find it harder to share. Agree who checks what and which choices require the leader or another department. Distinguish useful investigation from formal reporting that adds little, and notice whether people must repeat information already available elsewhere.
Connect the decision back to the team
Share the choice and its reasoning, and clarify which future signals matter. A risk that never becomes a problem was not necessarily unnecessary to report. Explore what you were able to choose because you heard about it in time. HR can ask whether actual responses support the invitation to report problems early.

Review what was knowable at the time and adjust the sharing criteria.
Can I ask for more information?
Yes; make its purpose, owner and timing clear while reviewing duplicate work.
